Preparing For Your First Visit
To ensure we have all the information needed to best care for you, please bring the following items to your first appointment:
A list of your medications.
Dental x-rays from your previous dentist (if taken within a year).
Your Insurance Information (if applicable).
Your Primary Care Physician’s name and contact information (and/or specialists you see).
During your visit, we will also spend some time reviewing your medical and dental history, any allergies or chronic health conditions we should be aware of.
